How do you ask for knife and fork in Chinese?

In most restaurants in China, forks and knives are not provided on the dining tables. If you find difficulty to use chopsticks, you will have to ask waiters to bring you the knife and fork.

You can say 请给我刀叉 (qǐng gěi wǒ dāo chā ), meaning please give me knife and fork.
